Registration is on a first come basis.
The registration process is not complete until a signed Waiver of Liability and Assumption of Risk form has been received by the Outdoor Resource Center. This form must be delivered to the Outdoor Resource Center or emailed to erin.compaleo@wright.edu in order to complete your trip registration. For more information, visit our website.
If the Outdoor Resource Center has concerns over a participant's Medical Information, a Medical Form, to be completed by a physician, may be required from the applicant to complete the registration process.
MEDICAL AUTHORIZATION In case of a medical emergency, I authorize Wright State University, or its duly authorized agents, to transport me to a health facility/hospital for medical care if it is deemed necessary. I further authorize such physician, health facility, or hospital to perform any emergency procedures necessary to provide me with medical treatment. Please indicate below your consent to these terms.
PHOTO RELEASE AUTHORIZATION I consent that any pictures taken of me during this orientation may be used in past and future archives of usable, promotional material at the sole discretion of Wright State University and its affiliates. Please indicate below your consent to these terms.
