Thank you for your interest in the Outdoor Resource Center's National Parks Road Trip: Utah's Mighty 5. To register, please complete the following form. This form serves as both application and registration, as well as applicable fee payment. For more information about the trip, click here. Student Rate is $599; Faculty, Staff, & Alumni Rate is $699. There is an option to register with a $100 deposit with the remaining balance due one month before trip departure.

Registration is on a first come basis. The registration process is not complete until a signed Waiver of Liability and Assumption of Risk form has been received by the Outdoor Resource Center.
MEDICAL AUTHORIZATION In case of a medical emergency, I authorize Wright State University, or its duly authorized agents, to transport me to a health facility or hospital for medical care if it is deemed necessary. I further authorize such physicians, health facilities, or hospitals to perform any emergency procedures necessary to provide me with medical treatment. Please proceed with registration to consent to these terms. If the Outdoor Resource Center has concerns over a participant's medical information, a medical form to be completed by a physician may be required.
PHOTO RELEASE AUTHORIZATION I consent that any pictures taken of me during this program may be used in past and future archives of usable, promotional material at the sole discretion of Wright State University and its affiliates. Please proceed with registration to consent to these terms.
TRIP POLICY You must review and agree to the Outdoor Resource Center Trip Policies in order to proceed with registration.
CANCELLATION POLICY Refunds are available if cancellation is requested more than 30 days prior to trip departure. No refunds within 30 days of trip departure.
